Glock gen 4 recoil spring assembly pushed through front of gun

Yesterday, I experienced the recoil spring assembly pushing through the front of my Glock 19 gen 4 (I've got it about a year, round count about 750), after cleaning it and re-assembling it.
I have never experienced anything like this.
The slide got stuck about 3/4 of the way, with the recoil spring assembly pushed through the front opening (of the slide).
Didn't know this was even possible.
Tried pushing the slide off but it was stuck.
pulled the trigger, and the slide came off.
Re-assembled and it assembled properly, gun seems to function, But I'm concerned about this.

Happened on my G43 as well.

Normally it happens if you don't have the metal circle part of the recoil spring properly seated as far down in the half moon section of the barrel.
I make sure to check it is seated well (don't look from the top when you reassemble it, it can be deceiving and make you think it's in the right position) before putting it together, no prizes for trying the assemble it as quick as possible.

Yeah make sure the RSA is properly seated in the half moon shape notch on the barrel lug.
Check that the RSA is parallel to the barrel before assembly.
