I'd really pimp the Google docs or Survey Monkey options. Google docs because its ridiculously easy and the data is available to pull and share.
SurveyMonkey because it's their core business and the security is pretty right.
If you continue with the current solution I'd just suggest a confirmation page after form submission.
And then other things like ensuring strong passwords to the db, user access etc.
Ok, there is data validation for the emails, the forms are customisable but I haven't checked out the options or documentation yet.
People don't need to log in as users, just submit. The main user account has a strong password, db will have a custom prefix and not the standard prefix and also the login-page will be custom/hidden and not default.
I suck with the WordPress site designs but we can always use the backend and do the rest custom with the help of quicksilver and those who know more than the rest of us :g:
I will check back here tomorrow morning and see how many test results was submitted and forward it to JS4.

Also a php form generator that can be used is php formMail Generator (http://formail-maker.com/generator.php) this is for a quick solution. Customize your form save and download the few php files and edit it as you like. Has admin panel section for form data and traffic ect which admin can download csv file.
Always best to make custom code for your needs.
