I carry two mags on my left appendix. For a while I had a fixed blade in a similar position to the one shown in the OP. What worked well for me was to put the tip in front of the mags. Since the mouth of the sheath was at normal belt line position, this made the butt of the knife tuck right up against my body. Mine was maybe a little bit more diagonal so it rode against the outside of my hip, so it wouldn't dig into my stomach if I bent forward. YMMV
