Bit of useless info.
Somehow my dad knew or met Mad Mike from the 50 ties - 60 ties I would guess.
As a child he once told me about Mad Mike - was a point or lesson he was trying to put across to me at some stage of my troubled youth. I forget the lesson, but recall he told me about Mad Mike and that he had a card in his pocket, on that card was a bloke standing against a firing squad wall with a row soldiers leveled off on him and the ranks hand up for a "fire on hand dropping" impression.
The man in picture against wall supposedly reflected Mad Mikes attitude to life - he had his fingers in his ears blocking them for the bang.
True - not true I do not know, but I decided there and then as a kid I wanted to be like that. Thought the guy being executed attitude was so mega flippant.
