Recently bought a Sako 243. Very nice rifle, but the first shot with the silencer is always 1/2 inch high and right.
Any advice how I can rectify this?

Sorry it does the same thing with or without the silencer. 1/2 an inch high at 50m.
Is it only for the first shot?
Is it the first shot after you have cleaned the rifle? It is a fouling shot, the first bullet rubs off on the high spots on the barrel. All subsequent bullets run on this fouling and will group consistently.
Yes, it's only the first shot. Not after I cleaned it, all the time!
The barrel is free floating, can it be that after the first shot, the barrel seats itself?
Is the rifle glass bedded? That might solve your problem

But, a Sako should not do this glass bedded or not.
Did you buy new or is it a pre-loved.
Check the tightness (or lack of it) of your action screws...
