I agree with your conclusion around inefficiency! Case in point - every time you submit - you need to provide 2 photos. However, the last 3 submissions I have made, the cards have come printed using the photo that i submitted in 2015.

The last time I had a newly submitted phot0 used was in 2011. Every card from subsequent apps and renewals has displayed the same 2011 photo.

Be there by 7:45 if you can, they only open at 08:00 if you're lucky... They've been very difficult lately.
